We are located at 3012 Summit Street, suite 2675, on the second floor of a medical office building at the corner of Summit Street and 30th Street. 

 

The building entrance is at 3100 Summit Street.  After COVID screening, turn right to walk down the interior corridor. When you reach the circular elevator space, you can either take the elevator up to the second floor or use a stairwell across from the elevators. 

 

Suite 2675 is right next to the elevator.
