Welcome, New Patients

  • Please fill out and return the Health History Form and Financial Agreement at least two days before your appointment.  

  • Please arrive 15 minutes early and provide your insurance card and ID.

  • We help with parking by providing 1-hour of free parking at the lot across from 3100 Summit Street. There is also metered parking available. Your visit may take longer than one hour.

  • Our building entrance is at 3100 Summit Street, where your temperature will be taken, and Covid-19 screening questions will be asked.

    • If you have cold symptoms, please call to arrange a telemedicine visit.

  • Our staff can also help obtain medical records from previous providers with a Release of Medical Information form.

  • Acupuncture insurance verification typically takes one week.

  • Masks are mandatory and all patients are asked to wash their hands upon entry and exit of our office.

  • No accompanying visitors are allowed at this time.

Appointment & Cancellation Policy

  • If you need to cancel or reschedule an appointment, call to notify us with at least 24 hours notice. A late cancellation or missed appointment will be charged a $50 fee.

  • Late arrivals will be seen only upon the discretion of the provider.

After-hour Phone Calls

  • Our regular business hours, Monday, Tuesday, Wednesday, and Friday, 8:00 am till 4:00 pm. We are closed for lunch between 12:00 pm and 1:00 pm.

  • If you call our office when we are closed, please leave a message and we will return your call the next business day.

  • If you have a matter that requires urgent medical attention, we advise that you visit the nearest Emergency Room.
